Protect the cervical spine either by a collar or manual stabilisation. Establish an airway (airway adjuncts, NP, Guedel, intubation w/ suction) if required and ensure that the patient is breathing. If not, bag. Continue the primary survey to assess for circulation and other disabilities e.g. broken bones etc. Assess pupillary reflexes to light and record a GCS. Examine for other signs of cerebral damage, any CSF leakage through the nose or ears or skull fractures etc.

Remove to hospital for urgent neurological assesment w/ CT head.
