The champion only loses the title under a title fight. There are different rules for title fights in many organizations. The champion can have a fight and it not be for the title. An example of this was the fight of Matt Hughes and Joe Riggs. Riggs did not weigh in at the proper weight. Hughes took the fight as a normal fight instead of a title fight.

In refering to world title bouts, a fight for the interm championship means: A champion (for instance a WBA champion) hasn't defended his title due to injury, so two contenders fight for the "interm" belt, meaning that whoever wins (the interm champion) has to fight the real champion when the "real" champion is fit and ready for action. Basically, it's just a money making think from the alphabet boys.

The champion is unable to defend his belt so a new champion is crowned. Since he never lost his belt the old champion is still champion. The new champion is considered a temporary champion and may defend his belt until the old champ returns to action. Usually the new and old champ unify the belts. Interim means temporary replacement
