When? Usually there's Russia, Chine, I think Japan, and some countries that used to be in the Soviet Union. Rhythmic Gymnastic is not that popular, because it isn't broaded as much as much as other type of gymnastic. But it's as hard, if you look on a professional level.

The ideal body for rhythmic gymnastics is slender and a little more long than for artistic gymnastics.The reason why people think rhythmic gymnasts are all skinny is because that is all we see.
