Canadian skater Vern Taylor was the first to land a triple Axel in competition at the 1978 World Figure Skating Championships. Midori Ito was first woman to land a triple axel in competiton at 1988 Aichi Prefecture Championship and then first in a major international competition at the 1989 World Figure Skating Championships I think it is Michelle Kwuan

There are 4 figure skating events: Men's Singles, Woman's Singles, Pairs, and Ice Dancing. With 3 medals( Gold, Silver, Bronze) for each of those events there are 12.

No. Rachael Flatt won the women's singles event at 2010 United States Figure Skating Championships and will be one of the two woman singles skaters selected to the team. Who will be the second skater on the Olympic team has not been announced but it will probably be either Mirai Nagasu, who finished second at the Championships or Ashley Wagner who finished third.
