Yes, fighting outside of the ring is completely legal. However, normal match types have a ten-count for wrestlers that are outside of the ring. If one wrestler is outside of the ring for ten seconds, the wrestler that is still in the ring wins.
The ref tells them at the start of the match, the matches are not scripted just the storyline is.

Wrestlers are going to fight and they will fight in wrestling matches. I love wrestling and wish someone knew but that wont happen till 2010 when Vince starts the new storyline(s). But we all know that HBK, HHH, John Cena, and Batista will dominate.
No, they actually fight by skill level. Weight is not factored into the competition.
Yes it is decided beforehand who will win the match but it is up to the wrestlers to decide on what happens in the match moves etc.
