Every school is different. Your school instructor will be able to tell you what you need to pass.
To receive a belt, you need to do an exam and your master needs to evaluate whether or not you know the "field survey". If so, you will receive a belt. The first one is yellow.

Depends on the system. Some Kung Fu systems and organizations use "sashes" instead of"belts" and the Gold or Yellow Sash is higher ranking than some that wear a "black sash". However, as far as I know with my 36 years of experience in Martial Arts I have come across no "belted" system where a yellow belt would ever out rank a black belt. Hope this helps.
If your a junior: Yellow belt + 1 stripe = 4th mon Yellow belt + 2 stripes = 5th mon Yellow belt + 3 stripes = 6th mon If your a senior it's called a 5th kyu :)
