I'd suggest a bicycle helmet. A good compromise between protection and comfort.
If you want more protection, use a motorcycle/scooter helmet. But it will be heavy and sweaty.
A skateboard/inlines helmet is an OK choice too.

Not usually. Some dealers will give you a helmet when you buy a bike, but nothing else.
Off road there is no law governing the use of a helmet, however, you would be foolish not to want to wear a good quality helmet. Your helmet is the number 1 piece of protective equipment you will own/wear.
Well, you definately have to have a helmet, a wingsuit and a parachute(obviously), but what else do you need?
Helmet-To-Helmet are not allowed. If that's what you were asking.
