most likely it means that a pitch is up in the strikezone (around the letters of the jersey of the batter) or low in the strikezone (around the knees of the batter).

(john here) I don't know why but major league umpires tend to call a lot of pitches high when they are actually still in the upper part of the strike zone.

It also refers to short "at bats" as in three up, three down.
