Ball out of bounds play provisional ball?

Yes, if you think you have hit your ball in play out of bounds you must play a provisional ball from the exact same spot as where you played the previous shot from. However, if you know the ball is out of bounds the next ball you play becomes the ball in play.

What happens in a game of golf when you lose the first ball you hit and then find it after you've hit a drop provisional?

You hit a provisional when the ball you just hit might be lost outside of a hazard. Usually that means you think you have hit it out of bounds or into some sort of mess where it's unlikely that you will find it. Hitting a provisional means that you are taking stroke and distance IF you don't find your first ball. If you find your first ball anywhere in play (whether or not you like where it is or even if it's unplayable) before you hit your provisional after reaching where the ball is likely to be, then the provisional is cancelled and you continue with your first ball, just as though you never hit the provisional. Once you've reached the point where your ball probably is and hit the provisional again, you're stuck with the provisional and the stroke and distance penalty that you took. This is covered in Rule 27-2, see www.usga.org. There are many interesting decisions related to this one, mostly about what constitutes a "lost" ball.

What rule governs teeing after lost ball?

Rules state that a lost ball costs you stroke and distance. In other words you add one to your score and go back to where you played the shot originally. If you hit a poor shot and think you may have lost your ball, most golfers will hit what is called a provisional. This means your provisional shot is in play if you can't find the first one you hit. This saves time and removes the necessity of returning to where you hit your shot originally. If you find your original shot you disregard the provisional ball. One addendum, on public courses in casual play, if no provisional ball was hit, most golfers will drop a ball near the spot they lost their ball, add a stroke and play on. Going back to the original spot and replaying your shot is time consuming and not really practical on a crowded golf course.

May a provisional ball be struck more than once in order to get it closer to the original ball position?

No

To make a golf shot on the green?

If you just hit a ball out of bounce from the tee box or if you don't know if you did you can call for a provisional ball. You would hit your second shot from the tee box "your provisional shot" and see if you can find the first ball. If you can't and your provisional ball is in play then you have play provisional ball and you accrue a 1 shot penalty stroke also. If you find your first shot off of the tee then you pick up your provisional ball and play your first tee shot. No penalty if you find your first tee shot and you can play it. So, the second shot from the tee box would be considered a provisional shot.

What are general bar rules when playing pool at the bar?

In the US, most bars post their "House Rules" which usually are a modified version of the American Poolplayer's Association (APA) Rules. The House Rules typically modify ball-in-hand rules, calling of a shot, and hitting the 8 ball. The link to the APA Rules is provided in the link below - Some examples of difference by House Rules are: APA Rules require calling the pocket, but not the entire shot - House Rules often require calling combinations, caroms, and banks. APA Rules do not consider the 8 ball "neutral", so it cannot be used as the first ball hit - House Rules may allow hitting the 8 ball first in a combination shot. APA rules require that you hit the 8 ball when it is your last object ball on the table, and a ball must hit a rail or be pocketed, or it is a foul - House Rules can call failure to hit the 8 ball loss of game.

How should you set up pool balls in a pool table game before starting?

This depends upon the game you will be playing and the rules you are playing. For 8 Ball, under BCA Rules, the 8 ball must be in the center with a ball to its left and right, and 1 stripe and 1 solid in the two rear corners. Under APA Rules, only the 8 ball position is required. It is customary, but not required, that the 1 ball is always at the foot spot. For 9 Ball, the 9 ball must be in the center and the 1 ball is placed at the foot spot. For Straight Pool, the two rear corners must be the 1 and 5 balls. Although many people have personal preferences, there are no other required positions for the balls.
