All players in a lobby are taken to a voting screen, where they discuss who to vote out. Once the votes are in, whoever has the most votes is ejected. Alternatively, if the players choose to skip or a tie is reached, nothing happens.
If you're a pitcher, just hit a couple of batters in a row and then you'll eventually either get ejected. Other than that, it just happens at random. My little cousin wanted to play so he was just pressing random buttons and Jon Lester got ejected for hitting like 5 players in a row.
Not usually, and if they do its not a big deal because all fines are tax write offs.

No, you cannot play with just 4. You must have 5 players on your team at the minimum to play. If you do not, it is an automatic forfeit.
If a team has all its pitchers ejected in a game, a position player will have to take the mound, assuming they have enough position players left to finish a game. Generally speaking, catchers have the strongest, most accurate arms of the position players, so I would assume the backup catcher would come in and finish the game to the best of his ability. If a team is unable to put at least 8 players on the field, the game is forfeited.
