It has happened several times, most recently by Adrian Beltre and Nelson Cruz of the Texas Rangers in Game 6 of the 2011 World Series.
Another notable back-to-back situation was in the first inning of Game 1 of the 1966 World Series, when both Frank Robinson and Brooks Robinson of the Baltimore Orioles hit consecutive home runs off Don Drysdale of the Los Angeles Dodgers. The blasts gave the Orioles an early 3-0 lead and they went on to sweep the Series.
Not many people realize it, but in the 1932 World Series, after Babe Ruth called his famous shot in Game 3, pitcher Charlie Root of the Chicago Cubs also threw a fat pitch to Lou Gehrig.
