In the save dialogue, select the appropriate extension or select "All (*.*)" and type the extension you want to save as.

The application must have the ability to save as that extension. For example, notepad cannot save as a JPEG image.

What is the meaning of file name and save as type when doing Save As in the file menu?

The File name is the name you want to give the file. The Save As Type allows you specify different formats that you can save the file in. For a web page saved in a text editor you would use .htm or .html for the extension, and use the *.* for the Save As Type to ensure it doesn't add a .txt extension onto the end of the filename.
