You get to the World Series by being the division champion. To get this, you have to win the most games in your division. Three teams get to the play offs. There is also a Wild Card in which the team with most wins in the league besides the division champion gets to be. The Wild Card also gets to go to the play offs. At the play offs, the eight teams verse each other in a first-to-win-three games series. The winner gets to go to the league championship series where there is a first-to-win-four games series. The winner of that gets to go to the World Series.
Only two teams go to the World Series: the American League champion and the National League champion.

American League teams play the National League teams for four series per season, in what is called "interleague play." also, in the World Series, an American League team plays a National League team.
If you're older than 12, it might be a little too late, but anyways, in order to get to the Little League World Series (I assume that's what you mean), you have to have the best record in your area, and then last as long as possible in a single-elimination bracket. The Little League World Series as a whole usually refers to the whole bracket, not just the final two teams remaining.
In 2004, the teams got their colors and they always wear that same color jersey.
The team that had the best won/loss record among all teams in each league won their respective pennant and advanced to the World Series.
