A pinch runner must be inserted for him at first base. Then, the batter that was hit cannot return, and the pinch runner must stay in the game (unless he's replaced by someone else, in which case the runner can't return).

Just because a batter spins to avoid the pitch and in doing so happens to move his bat across the plate does not mean that he swung at or attempted to hit the pitch - even if the bat passes completely over the plate. If the pitch is not in the strike zone and if the umpire judges that the batter was attempting to avoid getting hit rather than attempting to bunt at (or swing at) the pitch, he is hit by the pitch. The ball is dead and the batter is entitled to first base. If the umpire ruled that he was attempting to strike at the pitch when the pitch hit him, the ball is dead and the pitch is ruled a strike. No runners can advance. If the pitch is in the strike zone when the batter is hit, then the ball is dead and the pitch is ruled a strike. No runners can advance.

A substitute runner is allowed to finish the run. In MLB, rule 5.10(c)(1) states:

"If an accident to a runner is such as to prevent him from proceeding to a base to which he is entitled, as on a home run hit out of the playing field, or an award of one or more bases, a substitute runner shall be permitted to complete the play."

This rule covers both the batter and base runners. A situation similar to this occurred in a game between the Boston Red Sox and Toronto Blue Jays in 2005. Gabe Kapler of the Red Sox was on first base when Tony Graffanino hit a home run. Kapler went down with a ruptured achillies tendon between second and third base. Graffanino waited on second base while Kapler was loaded on a cart and taken off the field. Alejandro Machado entered the game as a pinch runner and finished the trot to home plate in Kapler's absence.
