A player is thrown out if one person from their team is trying to get an unfair advantage over the other team. This includes:

Moving the stick before the puck is dropped

Not placing the stick properly when requested to do so

Not placing the body square to the faceoff spot

Encroachment into the faceoff circle by a teammate.

When thrown out, another player from the team must take the faceoff, usually that person is not as skilled

In hockey, the official "penalty" that gets a player thrown out is called a "Game Misconduct". As the name implies, it is given to players who are guilty of some sport of unsportsmanlike behaviour. Sometimes this is repeated abuse of officials (verbal--physical is an automatic ejection), overly aggressive behaviour, etc. Hockey also has a "Match Penalty", usually reserved for players who deliberately attempt to injure an opponent. The penalty is ejection from the game and automatic suspension until the play is reviewed by the NHL.
