Well ... as phrased, that has never happened.
The Cardinals in Super Bowl XLIII and the Packers in Super Bowl XLV both won the toss and elected to defer their choice to the start of the second half. In both cases, their opponent elected to receive the opening kickoff. So, while the Cardinals and Packers won the toss and wound up kicking off, that was due to their election of deferring their choice to the second half which, in essence, gave the choice at the start of the game to their opponent.
