Either on the underneath side of the lid or on the back of the tank inside there should be a model number. Possibly on the underneath side of the tank also.
There are many different models but this is the one I could find closet to the $150 price range. GERBER Two Piece Elongated Toilet w/ 10" Rough-In 0021410 White, I hope that helps.

Based on what I've read, this typically occurs when it was not installed properly.
Inside the tank on the back side there are usually numbers stamped in. If not there, under the tank may be printed on ones.
Take off the back of the toilet. Look down to the bottom and you'll see a flap that is raised when you flush it. That flap is leaking. IF the toilet is old I strongly recommend the Gerber viper toilet. Inexpensive and works great.ANS 2 - change the flapper valve - takes about 5 minutes and costs less than $5

Inside the tank, usually on the back wall, there will be a number impressed into the porcelain. This is the tank number for a two piece toilet, or a model number for a one piece toilet. It's usually a four digit number, sometimes preceded with a K- (example: K-1234). Sometimes this info is repeated on the underside of the lid. Usually the lid also has a five digit number, which is the lid number. All of these numbers can help you find the replacement parts to repair your toilet.
Look inside the tank, and you'll find the serial number and possibly the brand...Then look up plumbing suppliers,and shop around.ANS 2 - The brand is almost always on the toilet, between seat and tank . The lid will often have 2-3 numbers inside it. Note them all and go to a major plumbing supplier, they will be able to track the number and order a lid.
