No. A jockstrap is made to be worn directly on your body, under your underwear. It would cause your underwear to bunch up or rub if worn over it. And it would not work at all if you tried to wear it over some styles of underpants.
This is completely up to you. If you wear boxers, normally you will put the jockstrap over the boxer or boxer brief. If you wear briefs you can wear it over or under. Anything smaller than that you might try wearing it under. Or you can just wear it as itself.

Not usually. Usually when you have a jock on, you don't wear anything else with it, just your athletic gear on top of it. However, if you are wearing compression shorts, sliding shorts, hockey shorts, etc., you should wear those over the jock. Some people (inexplicably) wear a jock under a swimsuit, too. If you're wearing the jock for running or track, you just wear your normal shorts over the jock, no underwear. You can wear a lot of modern underwear (briefs, gripper jams, etc.) INSTEAD of a jock, particularly if the underwear fits closely and is supportive.
A supporter or jock or jockstrap is put on like underwear, with the pouch covering your genitals.

It's definitely a good idea. Really a guy should wear a jockstrap or compression for all sports, especially if a cup is required (contact sports). Briefs might be ok if they are tight enough to support you, but boxers wouldn't be a good idea.
