I would recommend you wait until you start dischargingcolostrum from the nipples, then removal of the jewellery is not necessary. Changing the jewellery to PTFE* barbells (*flexible surgical Teflon) be a good option and would be simpler for cleaning and maintaining the piercings during breast feeding. Many of my clients have successfully maintained there nipple piercing using this method and have had no issues with breast feeding. Ultimately it's up to you and your comfort level.

You don't. That's just because most doctors are against piercings. You can always deny a ridiculous request like this and continue surgery with your nipple piercings in.

How long should you keep your earings in when you get your ears pirced?

If you intend on keeping the piercings, jewellery should be left in the piercings, there is no reason to remove the jewellery except to get rid of the piercings. Piercings can take a full year to become seasoned to the point where jewellery can be removed for extended periods.

How long will it take nipple piercings to get enough scar tissue to never close up?

There's no way to be sure honestly. Some people can have piercings for years that never fully heal to the point that they don't close. If you have other piercings that have healed to that point, you can use that as a gauge but otherwise, don't remove the jewelry unless you want to risk losing the piercing :)
