Assuming you mean defensive holding, the penalty is 10 yards from the basic spot and replay the down.
Note that if the ball is thrown, the "basic spot" is the line of scrimmage, so the 10 yards is enforced from the previous spot, regardless of whether or not the ball is caught. But if the QB tucks the ball and runs, the "basic spot" is the end of the run.
On the other hand, if you meant offensive holding and the hold occurs behind the basic spot, then the 10 yards are enforced from the spot of the foul.

In the NFL both feet MUST be in bounds. In high school one foot MUST be in bounds, and flag football I believe one, maybe two.
If the penalty requires that a down be replayed, then that down is still part of the quarter that has just ended. Therefore, the period is extended by an untimed down, so that the down can be replayed before the next period begins.

Same as in all levels of football: 5 yards for illegal substitution. The exception would be if 12 people actually participate in the play. That's illegal participation, which is 15 yards.
