If your ball is lost outside of a water hazard the penalty is stroke and distance. Return to the place that you last played from, drop (or tee, if it was a tee shot) and play from there. Count the shot that led to the lost ball and add one more, and count the replay.

Many beer leagues play drop it somewhere in the neighborhood of where the ball probably ended up, add a penalty stroke, and play on. Not USGA sanctioned, but on a public course where you have to get done before sunset it's a good way to keep the game moving.

It's a one shot penalty, and you must go back to where you originally played the shot from. That is, if you hit a tee shot and can't find it, you go back to the tee. In the case of a shot from the fairway or rough you go back to as close to where you played the last shot from as possible.

One shot penalty and you must go back to where you played the previous shot from, no exceptions. So if you hit a tee shot, and lose it in the trees, you take your penalty and play your third from the tee.

It depends on your definition of "one stroke". Take the following example:

You're on the tee and you hit your ball into the rough and you can't find the ball. Your original stroke is 1 stroke.

Your next "stroke" is moving your lost ball back to the tee box for your next stroke.

You would replace a ball on the tee and hit your 3rd shot.

It is a one shot penalty and you must replay the shot. Some 1- off the tee ( out of bounds), 2- penalty shot, 3- off the tee, the next shot is 4.

There is no limit, but when a player runs out of Golf balls, they are deemed unfit to continue, so they are disqualified.
