Here's how:

Each time the pitcher delays the game by violating this rule, the umpire shall call "Ball." The intent of this rule is to avoid unnecessary delays. The umpire shall insist that the catcher return the ball promptly to the pitcher, and that the pitcher take his position on the rubber promptly. Obvious delay by the pitcher should instantly be penalized by the umpire.

So the pitcher could do this enough times for a run to score, never throwing a pitch. That takes care of our run.

Now for the outs:

Either the pitcher can pick off all three guys on base in this inning where the guys get on base due to his charity and do this in other innings, too.

Or the batter can jump from one batters box to the other while the pitcher is ready but before he throws a pitch. This is called an out.

So all outs can be made without a pitch being thrown.

Theoretically, you could come into the game with nobody out and the bases loaded,. throw one pitch and get a triple play. Or, you could pick one guy off, then throw your first pitch (since pickoffs dont count as pitches) and get a double play. For that matter, you could just pick all three guys off and get out of the inning without actually throwing a pitch.

3-pitch innings happen fairly regularly, but I remember last April 15, 2008, Bobby Jenks came into a game with guys on 1st and 3rd and nobody out; then gave up a sac fly (1 out) then a double play (3 outs, one inning pitched) on just two pitches. My guess is that this is probably tied for the record.

There is no telling how many pitches can be pitched in a 9 inning no-hitter, however the least number is 27. One pitch per batter.

Correction:

True there is no maximum number of pitches thrown for a no-hitter, however there TECHNICALLY wouldn't have to be a minimum either, although, it would never happen in context, you could have all 27 batters in a game go to the plate out of batting order, once announced, they would be out, even without a pitch being thrown -- i know this is never going to happen, but within the rules it could be a situation to occur
