Since football championship games have been played in the NCAA (1992-1994 Bowl Coalition, 1995-1997 Bowl Alliance, 1998-present BCS) two teams from the same conference have never met for the national championship.

Yes. In the 1996 season, Florida State and Florida played in the Sugar Bowl for the National Championship. I'm not sure about any other intra-state meetings.
