Well,the baserunner on first might know that there is 2 out instead of 3 and tell the coach.In the Major League,a coach usually wouldn't make that mistake,in Little Leagues,the umpire would simply tell the kids to get back to their bases.

force to 3rd base means that when there are runners on 1st and 2nd and they are being forced to run to the next base ( in this case 2nd would be forced to 3rd) and all you have to do is step on the base, not having to tag the runner. this can also happen with a force to 2nd.

No. You answered your own question. The ball is dead if it comes into contact with the batter. Play should immediatly halt. I can see in softball, if the fourth ball is pitched and it hits the player, and is counted as a ball, then both runners would move. Of course if the runners on base are forced runs. Aside from that. I cant see a scenario where the runner would move bases based on a dead ball.
