You are not offside. The puck must enter the offensive zone for offsides to be called. Further, a player's skates can enter the offensive zone before the puck, provided the player is in control of the puck and no other offsides conditions are in effect.

When a player is offside where is the freekick taken?

Where the offside positioned player was at the moment their team-mate last touched the ball. If that spot is within the opponent's goal area, then the ball is brought straight out along a line perpendicular to the goal line to the "top" of the goal area (a.k.a. 6 yard line).

During a throw in in a football game an attacking player who was past to a defender gets the ball after the ball has been thrown and touched by the defender's head Is this offside?

You are not guilty of an offside infraction if you receive the ball directly from a throw-in by a team-mate, the defender deflection would still count as "the throw-in" unless they controlled the ball first. Even if they did control the ball, you are not guilty of an offside infraction if an opposing player passed it.

Can player be offside standing in the opponents net when a fellow player scores?

It is possible they are not. If the player that is in the offside position does not interfere with play (read: touch the ball), interfere with an opponent (make a move that causes an opponent to react), or gain an advantage (interfere with the vision or movement of the goal keeper, collect a rebound, etc) then there would be no offside infraction.

Was the Immaculate Reception a legal catch?

This is from the NFL rulebook:"Any eligible offensive player may catch a forward pass. If a pass is touched by one eligible offensive player and touched or caught by a second offensive player, pass completion is legal. Further, all offensive players become eligible once a pass is touched by an eligible receiver or any defensive player."Since the pass was touched by either an eligible receiver or a defensive player and then caught it is a legal catch.

If you are off the pitch when the ball is played are you offside?

For an offside infraction to occur a player must be in an offside position at the moment it is touched by a team-mate. If the goal keeper is from the other team then the player is not guilty of an offside infraction.
