Only once. On March 30, 2008, Kansas defeated Davidson to become the fourth #1 seed in the Final Four.
#1 North Carolina vs. #1 Kansas
#1 Memphis vs. #1 UCLA
Kansas defeated Memphis in the finals to win the 2008 NCAA Championship.

∙ 16y agoThe Selection Committee began ranking the #1 and giving one of the four a number one overall seed in 2004. Only once in those four years has a number one overall seed one the tournament- Florida in 2007.
In 2007, Florida was the number one overall seed and won the tournament.
In 2006, Duke was the number one overall seed and lost in the Sweet Sixteen to LSU.
In 2005, Illinois was the number one overall seed and lost in the championship to UNC.
In 2004, Kentucky was the number one overall seed and lost in the Second Round to UAB.

∙ 14y agoIt has happened once since 1985 when the tournament field expanded to 65 (64 after the play in game). That was in 2008 (#1 North Carolina West region, #1 Memphis South region, #1 Kansas Midwest region, #1 UCLA West region)
From 1979 to 1084 it did not ever happen, and previous to 1979 teams were not seeded.
