As far as I Know, the only player to die from an on ice injury during an NHL game was Bill Masterton of the Minnesota North Stars. In a game on Jan. 13, 1968, Masterton was checked and fell to the ice, hitting his head, causing a severe trauma to the brain. He died two days later on Jan. 15, 1968.
