The most common cause of death while Surfing is drowning caused unconsciousness due to an impact with rocks, your surfboard and other surfers and their boards. Hypothermia in colder waters without a wet suit may result in death but would probably lead to unconsciousness first, leading to drowning. Sometimes people may get snagged on the sea floor preventing them from surfacing and leading to drowning. In larger 10ft or more waves, or sometimes even less depending on circumstances, a surfer may fall off their board and be prevented from surfacing by repeated wave sets holding them down, this is rare but has resulted in drownings. Other than drowning someone may be killed outright by collisions.
Sea-life is sometimes a problem, with things like Jellyfish and sharks posing a threat to surfers, most shark attacks do not end in death but it is a possibility. Stings by jellyfish are rarely fatal unless anaphylaxis occurs or the string was particularly potent.

I can't say for exact, but in 2003 - 2004, 40 people died around Austraila and 17 around Hawii All I can say, is that surfing is a dangerous sport and if your going to try it, get information and lessons before-hand. : /
