Yes, if you are speaking about the flame that the Torchbearers are carrying. This is lit from the cauldron in Greece and next progresses on the relay.

If by chance the flames goes out due to wind, stumble or ? there is always a backup flame (usually carried next to the Torchbearer) to relight the flame. Typically an Olympic Torch Relay representative is nearby in case this happens.

Yes and no, the torch and the cauldron have gone out, but have always been reignited by a backup flame from the original source.

No. The flame is lit in a ceremony about 6 months before the Olympics. It is extinguised for the last time on the last day of the paralympic games.
