Yes, at least once. In April of 2000 a four-game series between the Red Sox and Indians was rained out due to five straight days of rain in Boston. The series was made up in September, when the two teams had to play eight games in five days, including three double-headers. (I was at all eight games! Unemployment has its good side.)
I know of several preseason games that were stopped and eventually cancelled due to thunderstorms. But the NFL will move the game to the 'away' team's stadium or move the game up a day if there is a hurricane coming. Back in 2004, the NFL moved the Dolphins home opener from Sunday to Saturday due to Hurricane Ivan. This will happen with hurricanes but I have never heard of a game cancelled due to rain or snow.
Update: On December 26, 2010. The NFL postponed the game between the Minnesota Vikings At the Philadelphia Eagles due to snow. It was pushed back until Tuesday. The Eagles were playing for a chance at a first round bye and with the game being pushed back until Tuesday, they came out flat and lost their chance of a first round bye, than eventually lost in the 2nd round of the playoffs.
It was Minnesota's second game of the season they played that was postponed.
Their game against the Giants on Dec. 12 was postponed a day after Minneapolis got 15 inches of snow. It was then moved to Detroit when the Metrodome roof collapsed. Their game against Chicago the following week was played at the University of Minnesota's outdoor stadium because of the damaged roof.

While numerous World Series games have been postponed by weather, by an earthquake (Game 3 in 1989), and by the 9-11 terrorist attacks (Game 1 in 2001), no Series game has ever been cancelled entirely.
I do not believe there has ever been one cancelled but they did move a Dolphins game from Sunday to Saturday due to the coming hurricane Ivan. The Eagles and Minnesota game in 2010 was moved from Sunday Night to Tuesday night due to a snow storm in Philly. It was the first Tuesday NFL game in over 60 years and the only one that I know of that was moved due to snow.

Game 3 of the 1972 World Series was about to start when hail began to fall at the stadium, when the grounds were already wet from an unusually wet fall that year. Bowie Kuhn -- the only person with authority to make weather decisions for World Series games -- ruled the field was unplayable.
There have been many World Series games played in frigid temperatures, most memorably in 1997 when the Florida Marlins played three games in Cleveland. But snow hasn't been a problem so far.
