I'll answer this question for Nascar back to 1954, Larry Mann died in September 1954 @ Langhorne, Pa up to the most recent, Dale Earnhardt died in Feb. 18, 2001 @ (Daytona 500), all together 32 drivers have died in Nascar, racing, qualifying, practice, flying, tire test, etc..

No you don't have to, but it's highly recommended. My race car didn't have a racing seat the first year I raced and I had racing belts. But it's much safer to have an actual racing seat, no matter what kind of racing your doing.
Buddy runs a driving (racing) school, and I don't know about now, but he was a test driver for Penske Racing as recent as 2008.
You can be a sports coach after doing this course.
