Massaging the breasts after breast implant surgery is highly recommended as it may reduce the chances of capsular contraction occurring, especially in cases where the implants have been positioned over the muscle.
The massage must move the implant from it's normal position to be effective, so it needs to be vigorous enough to achieve that but it shouldn't be so vigorous that it causes any bruising. The usual technique is to squeeze each breast firmly from the bottom upwards, forcing the implant to move higher than it usually lies, and this needs to be done for 2 minutes for each breast. As for the frequency with which this should be done, for the first week the implants should be moved every 3 hours, for the next 2 weeks they should be moved twice a day, and then once a day forever.
If your plastic surgeon instructs you to then, yes, you need to massage them. Every doctor is different and has different beliefs regarding the benefits of massaging. My plastic surgeon has me massage mine 3 times a day.
