If the ball completely exits the field across the goal line, and was last touched by a defender, then play is restarted with a corner kick for the opposing team.

Of course, if it exited below the crossbar and between the goal posts then a goal is awarded and the restart is a kick-off.

It depends, if the attacker hit the ball from the 'D' and it is deflected in the goal, then the goal counts and it is restarted from the halfway line. but if the attacker hit it from outside the 'D' then it is a 16 yard hit from the edge of the 'D'

If it does so between the goal posts and under the crossbar, then it is a goal for the attacking team and the restart is a kick off for the defense; assuming the defense was not performing a restart (goal kick, direct free kick, indirect free kick, etc).

If it exits the field across the goal line elsewhere then the restart is a corner kick for the attacking team.

After a goal the game is restarted in the same manner as a kick off at the start of either half. Both teams must be inside their own half, and the team who have just conceded start with possession of the ball at the centre spot. Only the two players who will take the kick off are allowed to be in the centre circle at this time. The referee will then blow his whistle once to commence play.
